Mail your Pre-Registration form and payment to Columbia Heights Preschool, Box 327, Galloway, Ohio 43119.

Enrollment is open to all 3, 4, and 5 year olds in the community whose birthday comes on or before October 1st of the year of enrollment. All children must be potty trained. No pull ups, please.
